In-Office Coverage Plans
Adult Plan – $425/Year
- Comprehensive exam for new patients.
- Two 6-month check-ups and two cleanings.
- Fluoride varnish for extra cavity defense.
- All routine x-rays (bitewings, PAs, panoramic).
- Emergency exams and x-rays during business hours.
- 15% off advanced treatments, including fillings, crowns, extractions, and implants.
- $500 discount on SureSmile clear aligners.
Periodontal Plan – $575/Year
- Comprehensive exam for new patients.
- Two 6-month check-ups and two cleanings.
- Fluoride varnish for extra cavity defense.
- All routine x-rays (bitewings, PAs, panoramic).
- Emergency exams and x-rays during business hours.
- 15% off advanced treatments, including fillings, crowns, extractions, and implants.
- 15% off deep cleanings (scaling and root planing).
Adding Family Members
- Children (ages 0-14): $325 per year.
- Family members (ages 15+): $375 per year.
Terms & Conditions
Enrollment & Coverage
- Non-refundable fees: The annual enrollment fee must be paid in full at sign-up and is non-refundable.
- One-year coverage: Your plan lasts for one full year and does not roll over.
- Scheduling responsibility: It is up to you to schedule and keep your appointments.
Plan Exclusions & Restrictions
- No combining with insurance: This plan cannot be used with other offers, discounts, or dental insurance.
- CareCredit discount adjustment: If you use CareCredit’s extended payment plan, the discount may be reduced by 5%.
- Fee adjustments: Prices are based on Infinity Dental Arts’ usual, customary, and reasonable (UCR) rates, which may change.
- No specialist discounts: Discounts only apply to services provided at Infinity Dental Arts and do not extend to referred specialists.
- Workers’ compensation exclusion: This plan does not cover treatment related to workers’ compensation.
- Exclusions on products: Discounts apply only to dental services and do not cover materials or products.
Appointment & Cancellation Policy
- Your responsibility: You are in charge of booking and keeping your appointments.
- Late cancellations: A fee may apply for missed appointments without 24-hour notice.
